The 4x4 aerobic interval format (four 4-min hard efforts at ~85–95% HRmax with active recovery) has been studied against continuous training and shown to strongly improve VO2peak and cardiometabolic markers, including in cardiac and metabolic-syndrome populations under supervision. It is time-effective and potent, but demanding. Cardiac or higher-risk individuals require clinical oversight.

Common misconceptions
- 'Intervals are only for athletes' (supervised versions help patients too)
